Work Order Form (Kuali)

  • Use this form to request repair/service for facilities and equipment maintained by Recreation staff.  
  • You will be directed to login via Single Sign-On
  • Please note that you only need to fill out the top portion of the form. Once you get to “Work Notes and Feedback” you no longer need to continue. There is a note letting you know when to stop.
  • Once you have filled out the requested information, please click submit on the top right
